Fee The cost for a SOPHAS application is $135 for the first school or program to which you apply. Any additional schools or programs to which you choose to apply will cost $50 per designation, even if you submit those schools or programs later in the application cycle. RL Applicants must provide a minimum of three letters of recommendation from academic advisors, professors, supervisors, mentors or anyone who knows the student in a professional context and is familiar with his or her academic and/or professional ability. Letters are submitted to SOPHAS electronically. Transcript The College of Public Health review and the Graduate College verification is based on your SOPHAS application. Transcripts should be submitted to SOPHAS at PO Box 9111,Watertown, MA 02471. No transcripts are required at the University of Arizona at this point. If you would like to expedite the process for the future, you may want to keep a second set of official transcripts in the original, sealed envelope.
